Hand in Hands is a support service for adults with Learning Disabilities, Autism Spectrum Disorder and/or behaviour that may challenge established services, to enable them to live as independently as possible in their own homes.

 

Our Vision is to enable all individuals to live fulfilled and valued lives, contributing to society and developing meaningful relationships which are free from barriers and discrimination and ensures they are treated fairly and with respect.

 

In order to achieve this, Hand in Hands:

 

  • promote individualised and Person-Centred support packages. 

 

  • support individuals to make informed choices, promoting positive risk taking, where necessary, to enable them to achieve their full potential, goals and aspirations.

 

  • work in an open, honest and transparent way to ensure the needs of the individuals we support are met in a way that keeps them safe from harm and fully explains any situations that may have not ‘gone to plan’.

 

  • provide as much, or as little support to individuals who live in their own homes, enabling them to be an active member of the community. 

 

  • support individuals to live the life they choose independently, in a way which fully respects their wishes, preferences, needs and beliefs. 

 

  • uphold individuals rights to be treated with respect, privacy and dignity. 

 

  • offer a flexible range of support services, based on the individuals needs, wants and aspirations including: personal care, domestic duties, banking and finances, shopping and engaging in social/leisure activities. 

 

  • encourage individuals to try new things and consider educational and employment opportunities in line with their interests and personal goals.

 

Our Mission is to offer individuals a high standard of support in their own homes and ensure those accessing our services will have the quality of life that we ourselves would wish for. 

 

Hand in Hands work with the individual, their families and others professionals to implement and review support plans and all concerned and regularly take part in an independent quality audit process which ensures the continuous improvement of service delivery.
